Please have your referring physician fax to our office your previous medical notes 48 hours prior to your appointment
If they do not, we may have to reschedule your appointment
Please arrive 30 minutes prior to your scheduled appointment time and be certain to bring:
You must obtain a referral from your physician (primary care or other referring physician) prior to your appointment.
If your insurance requires authorization for a visit to a specialist you must also obtain this authorization number prior to your visit.
Due to the high volume of telephone calls, we request that if you get our answering service,
please leave a complete message including your full name, a telephone number where you
can be reached, and your reason for calling. All messages will be answered before 4pm.
You must discuss your medication needs, including refill requests, with your physician
during your appointment. Please allow 48 hours advance notice for prescription refill requests.
In order to remain efficient in caring for you and your fellow patients, If you cancel your
Appointment less than 24 hours in advance, a $25 charge will be assessed.
If you fail to show up for your appointment on three occasions, you will not have the
opportunity to schedule another appointment.
Monday – Friday from 7:30am – 5:30pm
Life or death call 911, otherwise please leave a message with our answering service
(after hours). Your message will be relayed to our iSpine fellow who may contact for more
information and direct a plan of care.
Copays are due due at your appointment check-in. Cash, credit card, or check are accepted.

Please note: There will be a $30 fee for any returned checks.
Instructions for before your procedure:
Someone must accompany you home after your procedure. You may not drive. Please Wear comfortable clothing.
You may take your normal medications as prescribed. If you take pain medications on a
daily basis, continue to take them on the day of the procedure.
Your procedure may need to be postponed if you are being treated for or show signs of an
infection. Please contact us if you are taking an antibiotic or antifungal medication.
